| Children's Case Management |
The mission of the Children's Case Management program is to
promote a "better quality of life" for children and adolescents who
are emotionally disturbed or at risk of emotionally disturbance, by
providing continuity of care between inpatient and outpatient services.
The purpose of Children's Case Management is to reduce the risk of out-of-home /
residential placement by accessing community services in the least restrictive, most appropriate environment.
Psychiatric services include evaluation and medication clinic. Where appropriate, Case Managers work with
the Department of Children and Families (DCF) staff, school personnel, and other community resources to
ensure a comprehensive approach to the services provided for the child and family. |
